Adult Wellness Plan

2 routine cleanings every year
2 regular exams every year
All necessary x-rays throughout the year
Oral cancer and periodontal screening
2 urgent care exams per year (as needed)
1 whitening kit or discounted in-office whitening
25% off additional treatment
Child Wellness Plan
$21/mo or $249/yr
2 routine cleanings every year
Fluoride treatment with cleaning
2 regular exams every year
All necessary x-rays throughout the year

**This Dental Membership Plan is NOT dental insurance; it is a membership program for prepaid dental services offered by your dentist. If periodontal disease or gingivitis is present, additional treatment will be necessary prior to your cleaning. A periodontal maintenance plan may also be available for those who qualify. The Plan and any discounts included do not cover procedures performed outside of our practice.
